Copy this form if you are registering only part of the students from your district (or over 20 students).  Please share this information with other teachers who might be interested.  To maintain fairness, all students from the same school should attend the same contest date.  For the grade level, give the grade level of the test students will take.  Some sixth-graders may want to "test up" on MATHCOUNTS® material.  There could be other students who may want to "test up" at a higher grade level.  For those "testing up," awards will be given based on the level of test taken (that is, a sixth-grader taking the seventh-grade test should be entered as a seventh-grader and will be eligible for seventh-grade awards only).  In NO CASE may a student "test down" (for example, a fifth-grader can't take the third and fourth-grade test).
